- For frequency questions, choose Never, Occasionally, Often, or Regularly; for yes/no questions, select what applies.
- Your score updates automatically at the top of each section — a yellow or red result flags an area worth discussing in your consultation.
When you're done
If we're actively working together, send me your results so I can shape your protocol:

- Paste the score summary into the body of the email.
